Blue Shield of California ACA Plans 2026
Blue Shield of California is the second largest carrier on Covered California and by a wide margin the largest PPO carrier on the exchange. If you want a PPO in California, in most counties Blue Shield is the realistic choice. This page covers what they actually sell for 2026, which network reaches your county, and what changed this year.
Enrollment and share from Covered California’s March 2026 Active Member Profile, data as of June 1, 2026. Blue Shield’s own book splits 71.2% PPO and 28.8% Trio HMO. Across the whole California individual market including off-exchange, the California Health Care Foundation puts Blue Shield at 31%, essentially tied with Kaiser at 32%.
Read this first: 2026 is not a normal year
The enhanced federal premium tax credits expired December 31, 2025 and Congress did not renew them. California backfilled part of the gap with $190 million in state subsidies, but only for households up to 165% of the federal poverty level. Everyone above that is paying the unsubsidized or standard-subsidy price for the first time since 2021.
What that looked like in practice: new Covered California signups fell 32%. Among renewing enrollees who lost all of their enhanced credit, 22% cancelled, double the prior year. New enrollment above 400% FPL fell 59%. More than 130,000 renewing enrollees dropped down to a Bronze plan.

The two networks, and why this is the only question that matters first
Blue Shield sells individual coverage on exactly two networks in 2026. Every plan you can buy runs on one of them. Get this right before you look at a single premium.

Available in part of the county (20): El Dorado, Fresno, Kern, Kings, Los Angeles, Marin, Monterey, Nevada, Placer, Riverside, Sacramento, San Bernardino, San Diego, San Luis Obispo, Santa Barbara, Solano, Stanislaus, Tulare, Ventura, Yolo.
Not available at all: the other 30 counties, including Napa, Sonoma, Mono, Inyo and Imperial.
“Partial” can mean very partial. In Fresno County, Trio is offered in 3 of 102 ZIP codes. In El Dorado it is 4 of 27, in Solano 5 of 21. Note on names. Tandem PPO and Full PPO are Blue Shield employer group networks. They are not sold on the individual market. If a quote site shows you an individual “Tandem PPO” plan, it is wrong. See our Blue Shield group page for those.

There is no plan called “Bronze 60 Trio HMO.” The Bronze HMO is named Bronze 7500 Trio HMO and it is sold off-exchange only, which means no premium tax credit applies to it.

Who each plan is actually for
Silver 94, 87 or 73
Household income roughly 100% to 250% of FPL. These are the cost sharing reduction versions of Silver, available only through Covered California. Silver 94 has the lowest out of pocket costs of anything Blue Shield sells at any tier. If you qualify for CSR and buy Bronze or Gold instead, you are leaving real money behind.
Gold 80 PPO
You take regular medications, see specialists, or expect a procedure this year. Gold trades a higher premium for a $0 medical deductible on the Gold 80 PPO and much lower cost when you actually use it. Often the right answer for people who do not qualify for CSR but are not healthy either.
Bronze 60 HDHP PPO
The only HSA-qualified plan in the Blue Shield individual lineup. Healthy, self-employed, and want the tax-advantaged account. One California catch: California does not conform to federal HSA law, so your contributions are not deductible on your state return and the earnings are taxable annually. The federal deduction still works.
Trio HMO at any tier
You live in one of the 28 Trio counties, your doctor is in the Trio network, and you want a lower premium. Verify the doctor first, not after. You cannot switch out of an HMO mid-year because your specialist turned out to be excluded.
Minimum Coverage PPO
Under 30, or you have an approved hardship or affordability exemption. Catastrophic coverage only. Worth knowing: as of January 1, 2026, exchange-offered minimum coverage and Bronze plans are treated as HSA-qualified high deductible plans under the OBBBA change to the tax code, so this can now pair with an HSA.
Platinum 90
High, predictable medical use. A chronic condition with frequent specialist visits, an expected surgery, or fertility treatment. Rarely the cheapest total cost, but it is the most predictable, and predictability is worth paying for in some years.
What changed for 2026
- Blue Shield’s rate increase was 9.1%, below the Covered California statewide weighted average of 10.3%. For comparison: Kaiser 7.1%, Sharp 8.6%, Anthem 14.5%, Health Net 15.0%, Molina 14.7%, Valley 21.0%.
- Aetna left the California individual market entirely. Roughly 21,000 enrollees in Regions 3, 5, 6 and 11 were reassigned. If you were an Aetna member, Blue Shield PPO is available in your county, though Trio HMO may not be.
- The plan lineup did not change. We compared the 2026 and 2025 benefit summary indexes: same plans, same form numbers, same two networks. No new products, no withdrawals.
- Enrollment fell. Covered California ended open enrollment at 1,927,371 selections, down 2.6%. Blue Shield’s effectuated exchange membership went from 474,960 to 443,460 year over year, while its share of a smaller market ticked up from 24.6% to 24.8%.
How the money works
Premium tax credit
Reduces what you pay monthly, based on household income and size, and available at any metal tier. The enhanced version that ran from 2021 through 2025 is gone. The original ACA structure remains, which means the subsidy cliff above 400% of the federal poverty level is back.
Cost sharing reduction
Reduces your deductible, copays and out of pocket maximum instead of your premium. Silver plans only, roughly 100% to 250% FPL. This is the part people miss. A Silver 94 plan is a materially better plan than Gold for someone who qualifies, at a lower premium.
California state subsidy
For 2026 California committed $190 million from the Health Care Affordability Reserve Fund for state-funded credits, targeted at households up to 165% FPL. About 389,590 enrollees received one, averaging $45 per month. Above 165% FPL there is no state backfill this year.
Subsidies of any kind are only available through Covered California. Buying the same Blue Shield plan directly off-exchange means paying full price. We can enroll you either way at no cost to you.
Frequently asked questions
Is Blue Shield PPO worth it over Trio HMO?
It depends entirely on where you live and who your doctors are. In the 30 counties with no Trio at all, there is no decision to make. In a Trio county, the PPO costs more and buys you self-referral, out of network coverage, and a much larger provider list. If you have an established specialist relationship or you travel, that is usually worth it. If you are healthy and your primary care doctor is in Trio, it usually is not.
How do I check whether my doctor takes my plan?
Search by the specific network name, not by “Blue Shield.” A doctor in the Exclusive PPO Network is not necessarily in Trio HMO, and vice versa. Use the provider finder at blueshieldca.com and pick the exact network, or call us and we will verify before you enroll.
Can I still get coverage if open enrollment is over?
Only with a qualifying life event: losing other coverage, marriage, divorce, birth or adoption, moving, or an income change that affects your subsidy eligibility. You generally have 60 days from the event. Given how many people dropped coverage in January over price, income changes are worth checking on.
Does Blue Shield cover weight loss drugs?
Coverage varies by plan and by medical necessity, and it tightened for 2026 on the employer group side. For individual plans, check the drug formulary for your specific plan before you enroll if this matters to you. We will pull it for you.
Are Blue Shield and Anthem Blue Cross the same company?
No. They are separate, unrelated companies with different networks and different rates. There is no company called “Blue Cross and Blue Shield of California.” More on that here.
