Plan N is the lower-cost alternative to Plan G, and the trade is real: you take on office and emergency room copays and you lose Part B excess charge coverage. For a lot of people that trade is worth it. For some it is not. Checked against Blue Shield of California’s filed 2026 plan documents in August 2026.

What Plan N covers

Plan N is standardized, so the medical benefits are identical from any carrier. It pays:

  • Your Part A hospital deductible, $1,736 per benefit period in 2026
  • Part A coinsurance and hospital costs for up to 365 days after your Medicare days run out
  • Part B coinsurance, minus the copays described below
  • Skilled nursing facility coinsurance, $217 a day for days 21 to 100 in 2026
  • Part A hospice coinsurance and copayments
  • The first three pints of blood
  • Foreign travel emergency care, up to plan limits

The two things Plan N does not cover

1. Copays. Plan N leaves you a copay of up to $20 for an office visit and up to $50 for an emergency room visit. The emergency room copay is waived if you are admitted to the hospital. These are federal standardized amounts, not Blue Shield amounts.

2. Part B excess charges. A provider who does not accept Medicare assignment can bill up to 15% above the Medicare approved amount, and with Plan N you pay that yourself. Only Plan F and Plan G cover excess charges.

You also pay the annual Part B deductible, $283 in 2026, the same as with Plan G.

Who Plan N suits

Plan N tends to fit you if you see doctors a handful of times a year, you are comfortable checking that a provider accepts Medicare assignment, and you would rather keep the monthly cost down than eliminate every bill.

It tends not to fit if you see specialists often, in which case the copays add up quietly, or if you travel to areas where you cannot easily verify assignment. Run the arithmetic on your own visit count before deciding: at enough visits per year, Plan G costs less overall despite the higher monthly rate.

What Blue Shield adds

  • SilverSneakers at no additional cost, on Plan N as on all their Medigap plans.
  • A 7% household saving when two people 65 or over at the same address are accepted in the same benefit plan with coverage issued under one plan.
  • The Welcome to Medicare rate saving does apply to Plan N, for applicants within six months of enrolling in Part B. Ask us for the current amount.
  • Attained age rating, with the annual adjustment set at July 1 in Blue Shield’s 2026 documents.
  • DMHC regulation at 1-888-466-2219, with Independent Medical Review rights. Why that matters.

Switching later

Under California’s Birthday Rule you can move once a year without underwriting to a plan with equal or fewer benefits. Moving from Plan G down to Plan N qualifies. Moving from Plan N up to Plan G does not. Outside a guaranteed issue situation, such as your Medigap open enrollment period or a trial right, that move needs a Statement of Health and underwriting approval at any time of year.
