Plan G Extra is Blue Shield of California’s Plan G with a package of non-Medicare benefits attached. It is not a different Medigap letter, and on Medicare covered services it pays exactly what standard Plan G pays. Here is exactly what the “Extra” is. Checked against Blue Shield’s filed 2026 plan documents in August 2026.
First, what G Extra is not
Medigap letters are standardized by federal law. There is no standardized “Plan G Extra.” The medical benefits in G Extra are the standardized Plan G benefits, identical to Plan G from Blue Shield or from any other carrier. California permits carriers to attach new or innovative benefits to a standardized plan, and that is what the Extra package is.
So the decision is not “which plan covers me better.” On Medicare covered services, G and G Extra cover you identically. The decision is whether the extra package is worth the extra cost to you.
What the Extra package adds
- Vision through VSP: eye exams, frames, and lenses or contacts.
- Hearing through EPIC Hearing Healthcare: a hearing exam and hearing aids at tiered levels.
- Teladoc Health: 24-hour access to physician consults by phone or video.
- A quarterly over-the-counter allowance toward eligible drugstore items.
- Acupuncture and chiropractic as covered benefits rather than discounts.
SilverSneakers is included, but that is not an Extra feature. Blue Shield puts SilverSneakers on all five of its Medigap plans, including plain Plan G.
The regulator split worth knowing
Blue Shield’s Medigap contract is a Knox-Keene health care service plan regulated by the Department of Managed Health Care at 1-888-466-2219. The vision benefit inside G Extra is different: it is administered by VSP and underwritten by Blue Shield of California Life and Health Insurance Company, a separate entity regulated by the Department of Insurance at 1-800-927-4357.
In practice that means a dispute about a hospital claim and a dispute about a pair of glasses may go to two different regulators. Worth knowing before you need it.
The honest trade-off
Two things to weigh against the extras:
- The Welcome to Medicare rate saving does not apply to G Extra. It applies to Plan G and Plan N only. If you are new to Part B, that saving is money you give up by choosing Extra over plain G, on top of the higher base cost.
- Dental is not included. Blue Shield sells standalone dental plans alongside Medigap. Do not assume the Extra package covers teeth.
Our general view: if you were going to buy vision and hearing coverage anyway, the package can be worth it. If you were not, you are paying every month for benefits you may not use, and plain Plan G plus the Welcome to Medicare saving is often the better value. We will run both for you rather than guess.
You can move into G Extra later without underwriting
This is the part most people miss. Under California’s Birthday Rule you can change Medigap plans once a year without medical underwriting, provided you move to a plan of equal or lesser value. SB 407 excludes new and innovative benefits from that value test, and Blue Shield’s published materials apply it that way: a Plan G holder should be able to move to Plan G Extra during their birthday window without a Statement of Health. Because Blue Shield’s own birthday rule documentation is inconsistent in other respects, get this confirmed in writing before you rely on it.
So you are not locked out by choosing plain Plan G now. You can start on G, take the Welcome to Medicare saving, and revisit Extra at a later birthday. What G Extra covers on the medical side
Identical to Plan G: the Part A deductible of $1,736 in 2026, Part A coinsurance for up to 365 extra days, Part B coinsurance, skilled nursing coinsurance of $217 a day for days 21 to 100, hospice coinsurance, the first three pints of blood, Part B excess charges, and foreign travel emergency care up to plan limits.
You still pay the annual Part B deductible, $283 in 2026. No Plan G, Extra or otherwise, covers it.
